Statement of Past Enrollment

Submit an application letter through the Directorate of Academic Services by providing the following data:

1. Full Name.
2. Student ID Number.
3. Faculty/Study Program.
4. Place, Date of birth.
5. Full Address.
6. Contact.
7. Name of Academic Advisor.
8. Total number of SKS.
9. Target University / Institution

In addition to the aforementioned identity documents, applicants are also required to attach:

• Latest enrollment status from the Gateway system (printout)
• The Rector’s Decree regarding Resignation or University Transfer
