List of Documents for Academic Requirements

Below is a list of downloadable documents to support students in their academic activities:

Diploma legalization is generally available through the Academic Services Office at each faculty. Nowadays, alumni can conveniently access online legalization services through their respective faculty websites, as listed in the links below:

  3. Faculty of Civil Engineering and Planning
  4. Faculty of Industrial Technology
  5. Faculty of Psychology and Socio Cultural Sciences
  6. Faculty of Islamic Studies
  7. Faculty of Sciences
  8. Faculty of Medicine: Visit the faculty in person

For students who would like to apply for a diploma translation, a diploma replacement certificate, or a correction of inaccurate information on their diploma, the forms can be accessed below:

Requirements for Diploma Translation Application

  1. Fill out the Diploma Translation Application Form.
  2. Provide two (2) copies of a 3×4 cm color passport photo with a blue background (male students must wear a suit and tie).
  3. Submit a photocopy of the diploma.
  4. Provide proof of payment/transfer for the Diploma Translation fee of IDR 50,000.
    • Payment can be made at the Bank counter located in the Rectorate Building, or via transfer to Universitas Islam Indonesia’s bank account

Student Withdrawal Application

Below is the list of attachments required for a student withdrawal application at Universitas Islam Indonesia:

  1. The latest paid tuition fee receipt (SPP).
  2. Receipt of Catur Dharma settlement.
  3. Letter of Catur Dharma exemption from the Vice Dean of the Faculty (if the 4th installment of Catur Dharma has not been paid in full).
    Faculty Library Clearance Letter (specifically for FBE and FH students).
  4. University Main Library Clearance Letter.
  5. Photocopy of Academic Transcript signed by the Academic Advisor (DPA).
  6. Original Student Identity Card (KTM).

A certificate of transfer to another higher education institution will be available for collection within seven (7) working days after submission
